The problem

HAZOP and LOPA sessions produce their value in the room and lose much of it afterwards. Findings are recorded as prose, reconciling them with earlier studies is manual, and the reasoning behind a judgement is rarely preserved in a form anyone can query later.

• Follow an explicit analysis methodology rather than free-form summarisation
• Separate hazards, causes, consequences and safeguards as distinct records
• Track open actions and their owners across sessions
• Surface earlier analyses of comparable scenarios during the session
• Draft reports that the responsible expert reviews and signs off
